Medication

If the prescribed schedule of administration requires, the school nurse shall administer medication. The medication must be in a pharmacy labeled container and accompanied by a medical permission form signed by both the parent and physician.

Because of the age of Consolidated School students, all medications must be transported by a parent or guardian and delivered directly to the school nurse or other designated employee.  Downloadable form below.
